Anatomy of a lumbar disc herniation

How a worn disc at L4–L5 tears, releases its gel centre into the spinal canal, and compresses the nerve root that supplies the leg.

The disc is a pressurised cushion — a gel nucleus held under tension by concentric rings of collagen, sealed between the vertebral endplates.

With age and load the rings dry, weaken and fissure. The disc loses height and its back wall begins to bulge toward the canal.

A rip in the outer ring lets nucleus extrude into the lateral recess, narrowing the canal and flattening the traversing L5 root against the bone.

The compressed, inflamed root refers pain, numbness and weakness along the L5 distribution — down the leg to the top of the foot.
